What the report contains

A written clinical report: reason for referral, relevant history, mental status, standardized instruments applied (such as the PCL-5), their scores and the clinical interpretation. We document clinical findings; we do not give an opinion on the outcome of the case, and we do not claim the evaluation improves approval odds.
